School-Age Child CARE Programs
Before-School Program  The Boys & Girls Club of Oshkosh will offer a before-school program at Lakeside Elementary starting at 6:30 a.m. and going until 8:30 a.m. After-School Program The Club’s after-school programs run from school dismissal until 6:00 p.m. at the following schools: Roosevelt, Jacob Shapiro, Smith, Lakeside, Jefferson, Washington, and Merrill. Registration / Payments There is a registration fee of $25.00 for the first child, $15.00 for the second child, and $10.00 for the third. There are 32 openings for each program. However, the registration forms will be accepted on a first-come, first-served basis. Upon registration, the following must completed: 1. Boys & Girls Club Membership Application 2. Health History Form 3. Daycare Immunization Record 4. All forms must be completed to register. (You only have to fill out forms once, even if you are enrolling in both the before- and after-school program.) 2010 FEES:
Weekly Fees for Before-School Program $37 for the first child $24 for each additional child Weekly Fees for After-School Program $49 for the first child $35 for each additional child Additional Information The weekly fee will be charged regardless of how much the child attends each day/week. The program WILL operate on Early Dismissal Wednesdays for enrolled children only. Drop-ins are not allowed. Breakfast or Snack will be served each day and the children will have opportunities to work on homework, create an art project and play organized activities both indoors and outdoors.
